Untersuchung zur Qualitität der Bestrahlung von Patienten mit Prostatakarzinom durch Auswertung von Cone Beam CT Aufnahmen und Erstellung von praxisrelevanten Schlussfolgerungen für die Strahlentherapie

Quality irradiation Investigations of patients with carcinoma of the prostate and evaluation of cone beam CT recordings and creation of Applied coclousions for radiotherapy

  • Die mittels Cone-Beam-CT geführte Radiotherapie stellt eine Möglichkeit zur Beurteilung der Qualität der Bestrahlung von Patienten mit Prostatakarzinom dar. Ziel der Arbeit ist es, die intrerfraktionellen Organbewegungen von Blase, Rektum und Prostata auszuwerten und deren Zusammenhang zwischen Volumen und Dosis zu untersuchen. Hierfür wurden von 30 Patienten mit Prostatakarzinom jeweils 10 CBCT Datensätze im Bestrahlungsplanungssystem bearbeitet und ausgewertet. Die Studie zeigt, dass eine moderate Blasenfüllung sowie ein leeres Rektum die Chance des erwünschten Therapieerfolges verbessert. Aufgrund der durch interfraktionelle Varianz von Blase und Rektum verursachten Prostataverschiebung ist das regelmäßige CBCT zur Lagerungskontrolle sowie die Patientenaufklärung zu Ess- und Trinkverhalten während der Behandlungen ein wichtiger Bestandteil einer IMRT geplanten Strahlentherapie.
  • Radiotherapy with Cone-Beam-CT represents an opportunity to evaluate the quality of irradiation of patients with cancer of prostate. The analyses of the intrerfractionally organ movement of bladder, rectum and prostate and the proof of whose correlation between volume and dose rate are the objectives of the study. Therefor always 10 data sets from 30 patients with cancer of prostate in the treatment planning system were investigated. The study reveals that a moderate filling of the bladder and an empty rectum improve the required therapeutic success. Due to the movement of the prostate caused by interfractionally variance of the bladder and the rectum, the regular CBCT for positioning control and the patient information about eating and drinking behavior are an essential component of the IMRT planed radiotherapy.
