Process optimization in the outpatient rehabilitation center of SRH Wald-Klinikum Gera GmbH

  • Der derzeitige wissenschaftliche Stand belegt, dass die Arbeitsprozesse medizinischer Einrichtungen Ineffizienzen und verschenkte Ressourcen verbergen. Die monistische Finanzierung durch die Kostenträger, bedingt eine ständige Steigerung der Qualität und der vorzuweisenden Ressourcen. Um diesen Anforderungen standzuhalten ist eine kosteneffiziente Arbeitsweise unabdingbar. Wissenschaftler des Gesundheitsmanagements haben die aus der Industrie kommenden, Methoden zur Prozessanalyse, auf medizinische Gesundheitseinrichtungen angewendet und konnten herausfinden, dass gerade die administrativen Prozesse Effizienzpotentiale verbergen. Die Untersuchung der Prozesse fand am Beispiel eines Ambulanten Rehabilitationszentrum Anwendung. Mittels einer analysierenden Vorgehensweise konnte ein Überblick über die administrative Prozesslandschaft dargestellt werden. Die Vermutung, dass gerade diese Prozesse, Ineffizienzen verbergen, hat sich bestätigt. Die einzelnen Prozesse wurden durch den Autor der Arbeit visualisiert dargestellt und wörtlich beschrieben. Im Anschluss konnten somit Schwachstellen aufgedeckt werden, anhand derer dem Rehabilitationszentrum zunächst fünf essenzielle Verbesserungsvorschläge aufgezeigt werden. Diese Optimierungspotentiale beziehen sich auf einen gezielten Einsatz von Hilfsmitteln, beispielhaft wurden Checklisten erstellt oder die Anforderungen an den Einsatz einer Software erhoben. Teilweise konnten Aufgaben auf andere Mitarbeiter verteilt oder so strukturiert werden, dass sie nun in einer effizienteren Abfolge stattfinden. Die Prozesse sind nun transparent für alle Mitarbeiter dargestellt. Die Auflistung der dazugehörigen Verantwortungen und zu verwendenden Hilfsmittel, ermöglicht eine strukturierte Arbeitsweise, mit einer Früherkennung von Schwachstellen. Durch die schriftliche Festlegung der Aufgabenabläufe, ist die Grundlage gegeben, dass keine spontanen Handlungen mehr erfolgen sollten. Es hat sich herausgestellt, dass innerhalb von Aktivitäten, welche mit einem Patientenkontakt verbunden sind, zeitliche Ressourcen nicht ausreichend geplant werden können. Diese Arbeitsbereiche sind verschiedenen Störfaktoren ausgesetzt. Daraufhin wurden die Aufgaben umverteilt, sodass die Mitarbeiter einem geringeren Ablenkungspotential ausgesetzt sind.Das ambulante Rehabilitationszentrum resultiert nun von einer Handlungsanweisung, in welcher die Einführung der Prozesse Schritt- für-Schritt erläutert ist. Um die Veränderungen erfolgreich einzuführen bedingt es das Prozessverständnis aller Mitarbeiter. Somit ist es die Aufgabe der Geschäftsleitung den Mitarbeitern die Veränderungen transparent zu erläutern. Es sollten die Vorteile, welche durch die Umstrukturierung für jeden einzelnen Mitarbeiter hervorgehen, benannt werden. Ein Prozessverantwortlicher übernimmt die Aufgabe der Projektleitung und ist Hauptverantwortlicher für die Umsetzung. Zu Beginn werden die schon bestehenden Prozesse den Mitarbeitern näher gebracht, sodass im weiteren Vorgehen die optimierten Prozesse eingeführt werden können. Das Verständnis und Commitment (engl. Selbstverpflichtung) aller Mitarbeiter ist unabdingbar für das Vorantreiben der kontinuierlichen Verbesserung während eines angestrebten Veränderungsprozesses. Ist der Prozessgedanke in den Köpfen der Mitarbeiter verankert, werden sich zukünftige Veränderungen leichter einführen lassen.
  • The current state of scientific knowledge proves that the working processes of medical institutions conceal inefficiencies and wasted resources. The monistic financing by the cost bearers, requires a constant increase of the quality and the resources to be shown. In order to withstand these requirements, a cost-efficient working method is indispensable. Health management scientists have applied the methods of process analysis coming from industry to medical health facilities and have found out that administrative processes in particular conceal efficiency potentials. The investigation of the processes was applied to the example of an outpatient rehabilitation centre. An analytical approach was used to provide an overview of the administrative process landscape. The assumption that precisely these processes conceal inefficiencies was confirmed. The individual processes were visualized by the author and described literally. Subsequently, weak points could be uncovered, on the basis of which five essential suggestions for improvement were shown to the rehabilitation centre. These optimisation potentials relate to the targeted use of aids, for example checklists were drawn up or the requirements for the use of software were determined. In some cases, tasks could be distributed to other employees or structured in such a way that they now take place in a more efficient sequence. The processes are now transparent for all employees. The list of associated responsibilities and tools to be used enables a structured way of working with early detection of weak points. By the written definition of the task sequences, the basis is given that no more spontaneous actions should take place. It has turned out that time resources cannot be sufficiently planned within activities which are connected with a patient contact. These work areas are exposed to various disruptive factors. The outpatient rehabilitation centre now results from an instruction for action in which the introduction of the processes is explained step-by-step. In order to introduce the changes successfully, it requires the process understanding of all employees. Therefore it is the task of the management to explain the changes transparently to the employees. The advantages which result from the restructuring for each individual employee should be named. A process responsible person takes over the task of the project management and is main responsible person for the conversion. At the beginning the already existing processes are brought closer to the employees, so that in the further procedure the optimized processes can be introduced. The understanding and commitment of all employees is indispensable for advancing continuous improvement during a desired change process. If the process idea is anchored in the minds of the employees, future changes will be easier to introduce.
